Life happens. CANCELLATION/RESCHEDULE/NO-SHOW: You are able to reschedule/cancel your appointment 24 hours before your appointment. If you cancel or reschedule in less than 24 hours, the card that you booked with will be charged HALF OF YOUR TOTAL OF THE SERVICE BOOKED (for example.. your service is $100 you will be charged $50 for canceling/rescheduling outside of the 24hr window.) If you no show I will charge the card on file the same fee 50% of your booked service. LATE POLICY: If you are 15 minutes late it will be considered a no call, no-show and will be charged 75% of the scheduled service fee. If the card on file is declined, any future appointment cannot be booked until the fee is paid. WAIVERS/LIABILITY: Please fill out ALL forms prior to your appointment. I understand that pictures may be taken before, during, and after the appointment. If you do not agree to this term, please contact me before booking. I understand that if I have any concerns, I will address these with my esthetician. I give permission to my esthetician to perform the service/procedure we have discussed and will hold her harmless and nameless from any liability that may result from this treatment. I have accurately answered the questions above, including all known allergies, prescription drugs, or products I am currently ingesting or using topically. I understand my esthetician will take every precaution to minimize or eliminate negative reactions as much as possible. In the event that I may have additional questions or concerns regarding my treatment, I will consult my esthetician immediately. I agree that this constitutes full disclosure and that it supersedes any previous verbal or written disclosures. I CERTIFY that I have read and fully understand, the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold Aesthetically Summer, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this procedure, which may be affected by the treatment performed at the time of booking.
